Purpose:
To assess the distance between the posterior surface of square edge intraocular lens (I0L) optic and posterior capsule (PC) after standard phacoemulsification procedure with the use of intraoperative spectral-domain optical coherence tomography (iSD-OCT).

Methods:
Standard phacoemulsification procedure with IOL implantation was performed in 32 eyes of 30 patients. Intraoperative imaging was facilitated using iSD-OCT system Rescan 700, integrated into the surgical microscope OPMI Lumera 700 (Zeiss, Germany). Surgeries were performed by 3 experienced surgeons with concentration on careful washout of viscoelastic from behind the IOL. After one piece foldable acrylic hydrophobic IOL was implanted normal intraocular pressure was restored and controlled with Schiotz tonometry. The posterior chamber of the eye was scanned at the end of the surgery. By using graphic ImageJ software the distance between IOL posterior surface and PC was measured.
Results:
Intraoperative SD-OCT revealed the space between central part of I0L optic and PC in 92% which in average was 680 mcm (range 1100-260 mcm). The PC shape and curvature was similar to that before the surgery in phakic status. A contact of PC with the optic edge was noticed in 32% of cases. It was also shown that the contact of PC with IOL usually appears at the point of haptic derivation.
Conclusions:
Intraoperative SD-OCT facilitates the imaging of posterior chamber of the eye after standard phacoemulcification procedure. It was shown that even after careful viscoelastic washout the posterior capsule gets in contact in 32% of case only at the IOL edge site. However, the distance between central IOL optic part and PC remains in majority of cases.
