Abstract Details
Purpose:
To study and evaluate the outcomes of a new approach in managing congenital glaucoma visco trible (combined viscocanalostomy, trabeculotomy and mitomycin-C)
Setting:
Magrabi Eye and Ear Center, Riyadh, KSA
Methods:
In this study 11 eyes of seven infants (age range from 3dayes to 12 months only one age 6ys) 4 females ans 3 males . Viscocanalostomy is done with limbal based conjunctival flab after fashioning the superficial scleral flab Mitomycin-c was putted below the first flab and the bed then completing Viscocanalostomy as regular and injecting viscoelastic into schlem canal ostia then performing trabeculotomy leaving trabeculo-descemet window intact injecting viscoelastic in anterior chamber angle at site of trabeculotomy to tamponade it then closing the superficial scleral flab with 10/0 vicryl suture and closing the conjunctiva with 8/0 vicryl suture.
Results:
Mean preoperative IOP of 32.3 mm Hg (range, 25 to 44 mm Hg) decreased to a mean postoperative last visit IOP of 14 mm Hg (range, 11 to 18 mm Hg). Mean follow up period is 6Months-4Years.No intraoperative complication except for minimal controllable hyphema ,no hypotony or lost anterior chamber,no infection,follow up was 1 day,1week,2weeks monthly later on.only one case need surgical revision.one case need one medication
Conclusions:
VISCO TRIBLE (COMBINED VISCOCANALOSTOMY,TRABECULOTOMY AND MITOMYCIN-C) is an effective and safe technique in managing congenital glaucoma with no intra or post operative complication achieving satisfactory IOP reduction and good lasting effect
