Abstract Details

Purpose:

Add-on multifocal IOLs are designed for sulcus implantation to implement multifocality in eyes with monofocal IOLs. This study reports the first clinical and optical results obtained after implantation at the time of cataract surgery.

Setting:

Ophthalmic Unit A, University Hospital of Verona, Italy

Methods:

A series of 10 patients (20 eyes) undergoing cataract surgery received a monofocal hydrophobic acrylic IOL in the capsular bag, and a multifocal add-on hydrophilic acrylic lens in the ciliary sulcus. The add-on IOL has no base power, and +3 D diffractive add to provide near vision. Total length is 13 mm. The patients were followed for 2-9 months in the post-operative. Uncorrected and best corrected visual acuity, IOL centration, pupil status, optical aberration, and patient satisfaction were investigated.

Results:

Uncorrected visual acuity was 0.1 logMAR or better in every eye. Spectacle independence was obtained in all the operated patients in binocular vision. Centration was good in 19 eyes, however the IOL was 1.5 mm decentred in one eye, without affecting vision. Pupil anatomy was unremarkable. Corneal aberration was slightly increased by the 2-IOL system, from 0.19±0.03μ to 0.26±0.04μ at 4 mm aperture diameter. Coma showed similar pattern, but spherical aberration did not increase. Patient satisfaction was very high, with similar remarks as those for diffractive multifocal single optics IOLs.

Conclusions:

The implant of a multifocal add-on lens at the time of cataract surgery was safe and effective in this study. It provided the same results as primary multifocal implantation, with the advantage of reducing the number of IOLs required in the surgical theatre and the potential to be employed in eyes already pseudophakic.
