Abstract Details
Purpose:
To demonstrate how we used this technology to teach advanced surgical skills and the management of the complications of phacoemulsification surgery.
To demonstrate how an Ophthalmic Simulated Surgery Competency Assessment Rubric (OSSCAR) can be used to assess a surgeon's competence, before they move to live surgery.
Setting:
Cheltenham General Hospital, Cheltenham, UK
Methods:
We used advanced cataract eye models to simulate complications of phacoemulsification surgery;
Egg white was injected into the posterior chamber to simulate vitreous.
Posterior lens capsule was intentionally, or unintentionally torn with resultant “vitreous' loss, anterior vitrectomy was subsequently performed.
The “Zonules” were damaged to simulate a dialysis and a capsular tension ring was subsequently used to complete surgery.
Posterior chamber was overfilled with egg white to simulate increased vitreous pressure experienced with a suprachoroidal haemorrhage.
The OSSCARs were used to assess competence in managing these complications.
Results:
Virtual reality simulators such as the EYESi cannot simulate the complications associated with phacoemulsification surgery, nor can porcine eyes.
Simulated Ocular Surgery eyes can be used in any operating theatre, their use is not restricted to a wet-lab.
Simulated Ocular Surgery eyes enable surgeons to accurately simulate PCR, zonular dialysis and suprachoroidal haemorrhage, in their own operating theatres.
The OSSCAR scoring system allows trainers to judge a surgeon's competence before performing procedures, such as anterior vitrectomy, on patients.
Conclusions:
Simulated Ocular Surgery model eyes are effective and accessible teaching tools, in both straightforward and complicated cataract surgery.
