LASEK retreatment for postoperative ametropia after SMILE surgery

Abstract Details

Purpose:

To assess clinical results of LASEK as retreatment after SMILE corneal refractive surgery

Setting:

Retrospective case series

Methods:

We evaluated all retreatments performed after primary small incision lenticule extraction (SMILE) for correction of myopia and myopic astigmatism performed between 2012 and 2015 in two surgical centers by two surgeons (MB, RW). For retreatment of postoperative ametropia, laser assisted epithelial keratomileusis (LASEK) was chosen. Time between primary surgery and retreatment had to be at least 3 months.

Results:

Of 1523 eyes treated with SMILE, 11 eyes of 8 patients were retreated with LASEK using Mitomycin C (RW). Initial mean spherical equivalent of eyes retreated was -6.4 diopters. Persisting postoperative ametropia retreated was myopic astigmatism in 6 eyes (maximum spherical equivalent: -1.675 diopters) and mixed astigmatism in 5 eyes (maximum subjective cylinder 1.0 diopters). All LASEK procedures were performed without any intraoperative complications. Three months after retreatment no eye had lost CDVA, in 7 eyes CDVA remained unchanged, 3 eyes gained one line of CDVA and one eye gained two lines of CDVA, UDVA was 0.8 in 36 % of eyes and 1.0 in 64 % of eyes.

Conclusions:

Our retrospective study shows good safety and efficacy of LASEK as retreatment option for minor postoperative ametropia after primary SMILE procedure. However, one should anticipate that visual rehabilitation process in these cases is longer compared to primary LASEK surgery.
