Abstract Details
Purpose:
To compare reduction of low corneal astigmatism (1 to 2 cyl D) during cataract surgery by means of toric IOL versus limal relaxing incisions. Especially long term results were considered.
Setting:
Eye Clinic, Klinikum Ernst von Bergmann gGmbH, Potsdam, Germany
Methods:
10 patients with cataract and corneal astigmatism of 1 to 2 diopters on both eyes were included into this randomized intra-individual comparing study. One eye had a toric IOL (Tecnis Toric ZCT, AMO) implanted, the other eye was treated by limbal relaxing incisions (LRI) during cataract surgery and had a standard monofocal IOL (Tecnis monofocal ZCB00) implanted. Follow up was 3 and 12 months postoperatively. The following postoperative examinations were performed: visual acuity, contrast sensitivity, night driving ability and total corneal aberrations.
Results:
In both groups residual subjective astigmatism was -0.25 D (median) after 3 months. The toric IOL showed high degree of rotational stability (0°; 6°). The uncorrected distance visual acuity (UCDVA) was 0.2 logMAR (median) in the toric IOL group and 0.15 in the LRI group. Best corrected distance visual acuity (BCDVA) was 0.06 logMAR (median) for the toric IOL versus 0.0 logMAR for the LRI. Contrast sensitivity and night driving ability also showed no significant difference between the two groups. Total corneal aberration (RMS) for a 6mm pupil was 1.64 (0.74; 2.13) in the toric IOL group versus 1.47 (0.95; 4.14) in the LRI group.
Conclusions:
For the treatment of low astigmatism toric IOL seem not significantly superior compared to LRI. But in long term results (1 year) toric IOL seem slightly superior.
