Refractory malignant glaucoma after penetrating keratoplasty

Abstract Details

Purpose:

To report a clinical case of the rare malignant glaucoma after penetrating keratoplasty, which was refractory to all therapeutic options.

Setting:

Ophthalmology Department,Hospital Santa Maria – Centro Hospitalar Lisboa Norte, Lisbon Academic Medical Center, Lisbon, Portugal

Methods:

Case of a woman,70 years-old, submitted to penetrating keratoplasty of left eye(OS) on May 2015, due to a Fuchs dystrophy. There was not any intraoperative complication. The following day, athalamia was noted with seidel test negative and marked ocular hypertension. Maximum medical therapy was tried without success. Nd:YAG posterior capsulotomy and anterior hyaloidotomy, peripherical iridotomy were also tried without success. Pars plana vitrectomy with zonulo-hyalo-iridectomy was performed. At the end of the surgery the anterior chamber was formed. Athalamia recurred in the following day. The patient had no ocular pain. The visual acuity dropped from counting fingers to no light perception in 2 months.

Results:

Malignant glaucoma is a rare complication of penetrating keratoplasty. Nowadays, with the technical advances in vitreoretinal surgery, functional outcomes of this complication had increased, but there are rare cases refractory to all therapeutic options, despite early diagnosis and expedite and adequate treatment.
