Is DALK safer than PKP?

Abstract Details

Purpose:

To evaluate the complications of deep anterior lamellar keratoplasty utilizing the big bubble technique for various indications.

Setting:

Dar El-Oyoun eye hospital and El-noor eye hospital, Giza, Egypt.

Methods:

48 eyes of 45 patients underwent DALK using the big bubble technique. Intra - and post-operative complications were recorded. Follow up period was 1 year. The main indication was keratoconus , followed by corneal dystrophies and post-infection corneal scars. Intra-operative complications: Microperforation in 27 cases(39.5% ), Macroperforation in 2 cases (4.2%), Failure to obtain the big bubble in 5 cases (10.4%) Post-operative complications: Double Anterior Chamber that required intracameral air injection in 2 cases (4.2%), visually significant Descemet`s folds in 3 cases(6.3%), interface haze in 3 cases (6.3%),stromal rejection in 1 case (2.1%) , persistant epithelial defect in 2 cases (4.2%) No cases of endothelial rejection were recorded

Results:

DALK is a procedure with a considerable learning curve. Most intra- and postoperative complications are reversible but visual quality may be less favorable than penetrating keratoplasty.
