How to treat traumatic flap dislocation 10 days after accident

Abstract Details

Purpose:

To show our trouble shooting for the complicated lasik flap.

Setting:

KobeKanagawa Eye Clinic(KKEC), Tokyo, Osaka, Japan

Methods:

54 female received a succeseful lasik on both eyes in KKEC 13yrs ago. She got her right finger hit in left eye at the early morning, feeling something strange at first but went golf all the day because of sign getting better voluntally.

Results:

At 1st visiting our clinic almost accident 10days later, we found the twisted and slided out from original position flap with new epithelium recovered then a evident clear epithial sheet seen through the ophthalmic microscope. Best corrected visual acuity (BCVA) was 0.2(n.c.) From the topography, there was irregular astigmatism pattern at center cornea. Using surgical microscope and carefully debridement the regenerated epithelial sheet we could clearly set back the intact flap. At the post-op 3rd month check-up, with a fine hazy area under the upper hinge BCVA reached 1.0.

Conclusions:

To get the good vision again, using the manual epithelial scraper, we could identify original flap location, clean out neo-epithelium and reposition. FINANCIAL DISCLOUSRE: NONE
