Post small incision lenticule extraction (SMILE) interface fluid collection: a case of 'shifting ectasia'

Abstract Details
Purpose:
To report a case of interface fluid collection occurring after SMILE and to report an interesting finding of 'shifting ectasia' in this condition
Setting:
Centre for Sight, Banjara Hills, Hyderabad, India - a tertiary care super-specialty eye hospital
Methods:
Patient underwent SMILE for correction of compound myopic astigmatism in both eyes. He presented with decreased vision in his right eye two days postoperatively. Slit lamp examination, corneal topography, pachymetry and anterior segment ocular coherence tomography (ASOCT) was performed on successive visits
Results:
Right eye topography showed corneal steepening the location / axis of which kept on shifting on successive visits. ASOCT revealed presence of fluid in the interface. Patient responded to topical hyper-tonic saline drops and vision recovered to 6/6. IOP remained normal throughout without the addition of any antiglaucoma therapy.
Conclusions:
This is the first case report of interface fluid collection after SMILE. Interface fluid collection can occur without rise in IOP after SMILE. A new pathogenesis for the same is proposed. Also, a new pathognomic sign of 'shifting ectasia' is described . FINANCIAL DISCLOUSRE: NONE
