Iris-fixated intraocular lenses for the correction of aphakia in adult patients with the use of VacuFix device: 1 year follow-up

Abstract Details

Purpose:

To evaluate the results, the efficacy and complications of Artisan iris-clipped anterior chamber intraocular lens implantation in patients with aphakia with the use of VacuFix device.

Setting:

Department of Ophthalmology, Lister Hospital, East and North Hertfordshire NHS Trust, United Kingdom

Methods:

Medical records of ten adult patients who underwent Artisan IOL implantation for surgical correction of aphakia from 24/6/13 till 25/10/14 were reviewed in this retrospective, interventional, clinical case series study. Only patients with at least one year follow-up were included. Preoperative refraction, uncorrected visual acuity (UDVA), distance corrected visual acuity (CDVA) expressed as the logarithm of the minimum angle of resolution (logMAR), cornea status, ocular comorbidities and central retinal thickness (CRT) were analyzed preoperatively, one month and three months after surgery. The mean follow-up was 16 months. Aetiology of aphakia included subluxed crystalline lens, pseudoexfoliation, complicated cataract operation, ocular trauma.

Results:

The mean BCVA postoperatively was 0.54 in logMAR chart. 70% of patients had UDVA counting fingers preoperatively improved to 6/36 with pinhole. In 8 patients the CDVA improved in the first month postoperatively in a range from 6/36 to 6/12. Complications included corneal oedema (1 case) and high intraocular pressure (1 case). One patient presented with increased CRT>400 microns, but was a known diabetic on treatment for macular oedema with intravitreal ranibizumab injections. All the patients examined in the 3rd and 6th month appointment had improved vision ranging from 6/60 to 6/7.5.

Conclusions:

The presented results demonstrate that iris-claw lens implantation with the use of VacuFix device is a safe and effective method for the correction of aphakia in patients without capsule support. This surgical procedure has the advantages that it makes enclavation easier, it creates a perfect iris bridge, it is compatible with all phaco machines. Highly satisfying results were achieved in our study with low rate of complications. FINANCIAL DISCLOUSRE: NONE
