Spontaneous crystalline lens luxation into the anterior chamber after routine dilated examination

Abstract Details

Purpose:

To report an uncommon case of spontaneous crystalline lens luxation into the anterior chamber (AC) following a routine dilated examination of a patient with no medical history of eye injuries.

Setting:

State Ophthalmology Clinic, 'G. Gennimatas' General Hospital of Athens, Greece

Methods:

Interventional case report of a patient experiencing a crystalline lens luxation. Standard examination findings and a literature review analysis are described. The luxated lens was extracted through surgery.

Results:

A 60-year-old female patient visited our clinic due to visual impairment and pain in her right eye. According to her input, she had undergone dilated fundus examination the day before. During the dilation procedure, she experienced a sudden visual loss. Dislocation of the crystalline lens into the AC was reportedly diagnosed by her ophthalmologist and that finding was confirmed by us as well. The surgery, involved extraction of crystalline lens by incision at the limbus and placement of an AC IOL. No past medical history of ocular diseases, surgery, trauma or systemic disturbances was reported by patient.

Conclusions:

Spontaneous crystalline lens luxation into the anterior chamber is extremely rare, especially during a routine ophthalmic examination which involves pupil dilation. According to the literature, crystalline lens luxation is usually associated with several systemic conditions but no relevant evidence of such was detected. Moreover, general blood tests were normal, specific tests (e.g.: for homocystinuria) did not yield positive results and patient refused any further genetic tests. FINANCIAL DISCLOUSRE: NONE
