Abstract Details
Purpose:
To determine the effect of cataract surgery on vision and OCT changes in patients with active neovascular AMD (nAMD).
Setting:
Case series from Eastbourne District General Hospital, King's Drive, UK
Methods:
Retrospective case analysis of patients with uncomplicated cataract surgery whom were receiving active intravitreal injection (IVI) treatment for nAMD (within last 6 months). We excluded patients with other retinal comorbidity. The primary outcome measure was change in Best Corrected Visual Acuity (BCVA) following cataract surgery. Secondary outcome measures included changes in central retinal thickness (CRT), Volume (V) and signal strength (SS). The follow up period was between one to six weeks.
Results:
14,092 patients had cataract surgery over a period of 21 months, 56 had nAMD but 27 were selected as they had active nAMD. The mean age of patients was 86 years with 11 males and 16 females.
Mean BCVA showed significant improvement following cataract surgery from logMAR 0.8 ±0.4 to 0.6 ±0.3 (p=0.001). Signal Strength significantly improved from 5.5 ±2.2 to 7.3 ±1.5 (p=0.0002). CRT at 1mm showed an increase but it was not statistically significant (p=0.1, 208um± 81 pre to 234um ±50 post operatively). Retinal volume pre (8.3mm3 ±1.7) and post operatively (9.2mm3 ±1.3) showed significant statistical increase (p=0.02)
Conclusions:
Our analysis shows that although patients had improved vision following cataract surgery in context of active nAMD, retinal oedema measured using volume data increased and so we recommend close follow up of this group of patients after cataract surgery. Furthermore we found CRT at 1mm not representative of activity following surgery which may be important when future studies are considered. FINANCIAL DISCLOUSRE: NONE
