Retained ointment after phacoemulsification: new tools for diagnosis and therapy

Abstract Details

Purpose:

To present OCT findings of constant corneal touch and endothelial loss from retained ointment globules within the anterior chamber.

Setting:

University tertiary referral center.

Methods:

We present 2 cases of anterior chamber ointment with evidence of progressive endothelial cell loss. Anterior segment optical coherence tomography (AS-OCT) was used to evaluate the size, consistency, and proximity of the globule to the corneal endothelium.Gas chromatography-mass spectrometry was used to analyze the extracted globule in one case.

Results:

AS-OCT performed in both cases, showed similar findings to AS-OCT performed on in-vitro tobramycin-dexamethasone ointment; illustrating a homogenous well-circumscribed hyperintense lesion. Gas chromatography-mass spectrometry analysis documented the similarity to tobramycin-dexamethasone ointment in one case.

Conclusions:

Anterior segment OCT can help confirm the diagnosis of retained ointment globule. Corneal endothelial injury is a continuous process and its clinical manifestation relates to the size of the globule, initial endothelium count, and duration of ointment contact in relation to supine positioning.
