Sequential argon-YAG laser membranotomy of extensive persistent pupillary membrane with visual loss

Abstract Details

Purpose:

To report the successful use of sequential argon laser photocoagulation combined with neodymium: YAG laser in a bilateral case of broad thick base and extensive persistent pupillary membrane (PPM).

Setting:

Referral tertiary university medical center

Methods:

A 20-year-old female with best corrected visual acuity (BCVA) of 20/50 in both eyes diagnosed with extensive PPM. YAG laser was used to lyse the fine avascular strands at first. However, the greater component of the PPM was vascular, thus Argon laser photocoagulation was performed at the junction of the PPM and the iris collarette. Subsequently, additional neodymium: YAG laser allowed severing of the strands of the PPM from the iris.

Results:

These laser procedures resulted in shrinkage of the embedded vessels and the PPM without hemorrhage in the iris or any other complications. A central membrane remained adherent to the anterior capsule after completion of the laser procedures. Mild pigment dispersion was found in the anterior chamber without associated hemorrhage. One day, one week and one year follow up after laser therapy, intraocular pressure was normal with no hyphema or anterior chamber inflammation. Best-corrected visual acuity improved to 20/30 in both eyes.

Conclusions:

We conclude that combined sequential argon-YAG laser membranectomy can effectively be used to treat broad thick base PPM without bleeding of the iris or other major complications. FINANCIAL DISCLOUSRE: NONE
