Optical blending of high order aberrations in extreme irregular astigmatism

Purpose:
Strategies and solutions correcting optical aberrations in extreme irregular astigmatism cases, using reversible modalities when surgical strategies are not chosen.

Methods:
Irregular corneas present extreme values of mostly asymmetric astigmatism. Indicative cases are presented, categorized using Oculus Pentacam, which correction is essential to provide functional to optimal vision. The prevalence of the optical irregularity depends on front corneal surface shape. A recently evolved scleral lens design has been used, correcting low and high order aberrations of the corneal surface. The novel Epsilon scleral lens design, incorporates multi aspheric surfaces, toric periphery to accommodate toric scleras and front toric option. Aberrometric case analysis is presented using COAS wavefront aberommeter.
Results:
Adequately designed and fitted scleral lenses, successfully correct irregular corneas. Optimal correction depends also on back corneal shape as well as on lens centration. Challenging cases can be treated, following the basic scleral lens principles as absent of corneal touch and adequate limbal clearance.
Conclusions:
Scleral lenses tend to become the first choice of correction for the cases of irregular astigmatism, providing initial comfort, corneal surface respect, stem cells limbal region preservation and quality optical correction. Due to theis scleral landing are indicated even in cases that patients move in demanding environments.
