Deep anterior lamellar keratoplasty and simultaneous implantation of intrastromal corneal rings in the graft: intraoperative prevention of corneal astigmatism in cases of keratoconus

Purpose:

Decrease corneal astigmatism after keratoplasty in patients with keratoconus

Setting:

The Ophthalmology Department of Federal state Institution Clinical Hospital of the Presidential Administration of the Russian Federation and Moscow State University of Medicine and Dentistry Moscow, Russian Federation.

Methods:

The analysis included 10 patients (10 eyes) who had undergone deep anterior lamellar keratoplasty (DALK) for keratoconus patients. In all cases femtosecond laser was used for forming the graft and the recipient bed, and the intrastromal tunnel with a diameter of 5 mm into the graft. During keratoplasty in the tunnel an open-ended corneal ring (3590) made of polymethylmethacrylate (PMMA) with a diameter of 5 mm was implanted. The diameter of the graft was 8 mm. Follow - up: 6 months.

Results:

In all cases the postoperative course was standard, like a regular DALK. The graft was transparent. The implant was in the correct position. We observed a significant improvement in corrected (0.75±0.23) and uncorrected distance visual acuities (0.52±0.34). Postoperative astigmatism in the first month after surgery was 1.5D±2.76, after half a year 1.1D±0.45).

Conclusions:

The proposed operation is an effective method of intraoperative prophylaxis in keratoplasty ametropia. Simultaneous implantation of intrastromal corneal rings in the graft for deep anterior lamellar keratoplasty prevents the development of high degree of astigmatism in patients with keratoconus in up to 6 months. An analysis of the results after removal of corneal sutures in remote periods of observation is planned. FINANCIAL DISCLOUSRE: NONE
