Abstract Details
Purpose:
To report a contemporary improvement of a novel idea, designed to replace traditional graft suturing in perforating keratoplasty.

Methods:
The modified TR-Ring is placed intrastromally into the patient cornea before the diseased cornea is removed. Turning the TA -Device clockwise will lock the device into the peripheral cornea. Once the diseased cornea is removed the pregrooved graft is inserted into the device. By turning the graft clockwise it is permenantly locked into its final position. The Procedure is demonstrated in animation.
Results:
This 'jewelry insertion' type of surgery is the world's first attempt to leave a completely unsutured optical zone from day 01 after penetrating keratoplasty.
Conclusions:
The many theoretical advantages (including the possibility of introducing a completely cornea supported IOL among others) but some important disadvantages argue for continuation of further experimentation and refinement of the technique that has the ambition to extend the laser trephination precision to a new level of clinical significance FINANCIAL DISCLOUSRE: One or more of the authors has significant investment interest in a company producing, developing or supplying product or procedure presented
