Study with Pentacam of the effect of hemodialysis in the biometric characteristics of the cornea and the corneal thickness and in the intraocular pressure

Abstract Details

Purpose:

To study the differences in the intraocular pressure (IOP), the Central Corneal Thickness (CCT) and in several biometric parameters of the cornea (eg.K1,K2) in patients with renal failure undergoing hemodialysis (HD), before and after the procedure.

Setting:

2nd Department of Ophthalmology, University of Athens, “Attikon” Hospital, Athens, GREECE

Methods:

27 patients undergoing hemodialysis had Visual Acuity (VA) measurement, IOP measurement by Goldmann tonometer, ultrasound CCT pachymetry and examination with Pentacam (Oculus) for K1 and K2 measurements, corneal thickness topography and anterior and posterior surface evaluation. The results before and one hour after the hemodialysis procedure were compared. Each patient was measured 3 times in 3 different visits. Statistical analysis was performed separately for the right (RE) and the left (LE) eyes of all patients with the Statistical Package for Social Sciences (SPSS). The Wilcoxon for paired samples test was used.

Results:

There was no statistically significant changes found in VA, CCT and all the basic biometric parameters mentioned above, before and after hemodialysis. The mean CCT before and after hemodialysis was 569.2 and 568.1 respectively for the RE and 572.8 and 571.9 for the LE, but the decrease was not statistically significant. Mean K1 was 44.1 and K2 was 43.4 in both eyes before and after HD. The overall IOP before and after the procedure did not show any statistically significant change, although in one visit, statistically significant decrease was observed in IOP after hemodialysis compared to IOP before.

Conclusions:

It is the first time that all biometric parameters of the cornea, examined with Pentacam, were compared in patients undergoing hemodialysis and according to our results there was no significant change. According to other studies CCT decreases immediately after hemodialysis. In our study, measurements showed small decrease, not to statistical significance. There are controversial reports whether hemodialysis increases IOP or the opposite. In our study the IOP did not show overall statistically significant change FINANCIAL DISCLOUSRE: NONE
