Corneal changes with different RGP lens fittings after riboflavin-UVA corneal collagen cross-linking (CXL) in progressive keratoconus

Purpose:
to evaluate the safety, efficacy and possible additive role of different fitting philosophies of RGP lens following riboflavin-UVA corneal collagen cross-linking (CXL) in management of progressive keratoconus.
Setting:
Mansoura Ophthalmic Center, Mansoura University, Egypt
Methods:
The study included 36 eyes without RGP lens and 39 eyes were randomly fitted to either apical touch, or three points fitting approaches from 2 to 10 weeks post-CXL. None of the patients had been wearing contact lens (CL) before CXL. The effect of the CXL without and with RGP lenses with different fitting philosophes was measured by the change from the baseline of best spectacle corrected visual acuity (BSCVA), refractive sphere and cylinder, maximal keratometry value (K max), simulated keratometry, astigmatism, thinnest corneal thickness and I-S value. The results of these measurements were reported pre-CXL and one-year post-CXL.
Results:
In the eyes without CL, none of the study parameters showed a significant change from baseline. The 2 groups of eyes fitted with RGP lenses post-CXL showed a significant improvement in several parameters including refractive cylinder, I-S value, K max, simulated keratometry, thinnest corneal thickness and astigmatism. These changes were more marked in the group with flat fitting RGP lenses.
Conclusions:
RGP lenses wear can result in significant improvement of topographical parameters following CXL. Apical touch fitting is a safe approach associated with greater corneal changes in comparison to three-point touch lens wear. FINANCIAL DISCLOUSRE: NONE
