Differences in energy expenditure for traditional and femtosecond cataract surgery using two different phacoemulsification systems

Abstract Details
Purpose:
To compare the mean cumulative dissipated energy (CDE) in patients undergoing cataract surgery using 2 phacoemulsification platforms with either femtosecond laser-assisted pretreatment or traditional phacoemulsification.
Setting:
Bascom Palmer Eye Institute, Miller School of Medicine, Miami, Florida, USA.
Methods:
The study included consecutive patients underwent cataract surgery by 2 surgeons at Bascom Palmer. Patients underwent surgery using either femtosecond laser-assisted cataract surgery (FLACS) (two platforms were used, Catalys or LenSx), or traditional phamoemulsification. Phacoemulsification was performed using either the Centurion or Infiniti phaco machines. Mean CDE was recorded in all procedures.
Results:
Study included 733 eyes of 710 patients. Eyes were divided into 3 groups (Catalys, LenSx and traditional) each having 2 subgroups (Centurion or Infiniti). CDE was significantly lower when using the Centurion as compared to the Infiniti in all groups and for all cataract grades (p=0.014 for grade 1, p<0.001 for grade 2, p<0.001 for grade 3, p=0.002 for grade 4). With FLACS cases, CDE was higher in the Infiniti group compared to the Centurion group for all cataract grades, however it was only significant for grade 2 cataracts (p=0.043 for Catalys group, p<0.001 for LenSx group).
Conclusions:
The Centurion active fluidics and balanced tip design seems to reduce CDE values when compared to the Infiniti (gravity fluidics) phacoemulsification platform for traditional cataract extraction. FLACS pretreatment with either Catalys or LenSx further reduces CDE, independent of cataract grading. FINANCIAL DISCLOUSRE: NONE
