Evaluation of an intra-operative toric lens alignment system

Abstract Details

Purpose:

To evaluate a dedicated operating microscope with augmented reality technology to allow intra-operative toric intraocular lens (IOL) alignment without preoperative corneal marking and to show rotational stability of different lens types 1h post operatively.

Setting:

Vienna Institute for Research in Ocular Surgery, Department of Ophthalmology, Hanusch Hospital, Vienna

Methods:

This study included patients scheduled for cataract surgery and receiving a toric intraocular lens. At the pre-operative visit optical biometry was performed (IOLMaster 500, Carl Zeiss Meditec AG, Germany). During optical biometry a red-free image of the eye was taken automatically. During surgery, a recently developed set-up was used for toric IOL alignment. (OPMI Lumera 700 and Callisto Eye, Carl Zeiss Meditec AG, Germany). The set-up allows to set the planned IOL orientation and the surgeon can see the planned orientation of the toric IOL as well as the horizontal axis in the eye piece using augmented reality.

Results:

In total, 50 eyes of 50 patients were included. No adverse events were reported and feasibility of the intra-operative marking was high. Deviation between the intra-operative and aimed IOL axis was -0.12° (SD: 1.47°). Between the one hour post-operative and aimed IOL axis the deviation was 3.32° (SD: 5.93°).

Conclusions:

Intra-operative toric lens alignment is an accurate and fast procedure resulting in precise toric lens aligment and simplifies positioning of toric IOLs FINANCIAL DISCLOUSRE: NONE
