Late in-the-bag intraocular lens dislocation in eyes with high myopia

Abstract Details

Purpose:

To assess preoperative features, frequency, surgical approaches and outcomes of late in-the-bag dislocation of posterior chamber intraocular lenses (IOLs) in eyes with high myopia.

Setting:

A retrospective study.

Methods:

Fifty three patients (53 eyes) were enrolled. Inclusion criteria were patients with pre-existing high myopia who underwent surgery for late in-the-bag IOL dislocation between November 2009 and April 2015. Medical records were reviewed. Data from before and after secondary surgery were analysed.

Results:

The time interval between phacoemulsification and the diagnosis of IOL dislocation was found to be 68,7 ± 16,5 months. The increase in frequency within the reviewed period was statistically significant. The dislocated in-the-bag IOL was replaced with a scleral fixated IOL in 43 (89,2%) eyes and iris-fixated IOL in 2 (1,3%) eyes. Five (9,5%) eyes were operated for IOL reposition. Finally, 3 (3,2%) eyes were left aphakic. Mean BCVA improved significantly after surgery (p<0,05). Complications occurred significantly more frequently during IOL exchange/removal surgery when compared with scleral suturing (p < 0.0001).

Conclusions:

The frequency of late in-the-bag dislocation of posterior chamber IOLs in eyes with high myopia increased during the observation period. Scleral fixated IOL implantation provides significant increase in postoperative BCVA and seems to be more reliable in terms of intraoperative complications. FINANCIAL DISCLOUSRE: NONE
