Anterior basement membrane corneal dystrophy and cataract surgery with implantation of trifocal intraocular lens in practice : case reports

Purpose:
We present two cases of patients with cataract and anterior basement membrane dystrophy who underwent cataract surgery by phacoemulsification and implantation of premium trifocal intraocular lens.
Setting:
European Eye Clinic Lexum, Prague, Czech Republic
Methods:
The medical records of 2 patients (4 eyes) with anterior basement membrane dystrophy and cataract surgery with implantation of trifocal intraocular lens were retrospectively reviewed. In the first patient the central corneal epithelium (8 mm) was debrided with a Beaver blade to remove the abnormal epithelium which induced irregular astigmatism in the area of visual axis. Two months later, when the corneal surface was regular and stable according to topographic map, phacoemulsification with implantation of trifocal intraocular lens was performed. In the second patient no scraping of the epithelium was done because the corneal topographic map showed no irregular astigmatism.
Results:
The preoperative decimal binocular visual acuity in the first patient improved from 0.2 to 1.0 after surgery, near visual acuity was Jaeger 1, intermediate vision J6. In the second patient binocular visual acuity improved from 0.5 to 1.0, near visual acuity was J1, intermediate vision J5. Both patients were satisfied with their postoperative vision. During the follow-up period of 3 years in the first patient and 1 year in the second patient neither recurrence of corneal erosion nor recurrence of map-dots-finger prints in the cornea were noticed.
Conclusions:
Implantation of trifocal intraocular lens in patients with anterior basement membrane dystrophy not always requires corneal debridement. After careful patient selection, in case when the irregular astigmatism affects the visual axis, the combined procedure (the removal of corneal opacifications to achieve regular keratometric values and then cataract surgery) can be considered to solve the two problems. This consequently allows implantation of premium trifocal intraocular lenses. FINANCIAL DISCLOUSRE: NONE
