Abstract Details

Purpose:

The objective of present study was to explore the potential beneficial impact of a contemporary artificial tear preparation on postoperative discomfort following cataract extraction surgery.

Setting:

Department of Ophthalmology, University Hospital of Alexandroupolis, Greece

Methods:

120 participants with stage 2 senile cataract and dry-eye-disease (DED) severity up to grade 1, were randomly divided in two groups: 1) Control Group received fixed combination of tobramycin-dexamethasone (FCTD) for 3 weeks (Tobradex, Alcon) and 2) Study group received FCTD for 3 weeks and polyethylene glycol 400/propylene glycol/hydroxypropyl-guar (Systane Ultra, Alcon) for 6 weeks. The same surgical procedure was applied to all patients by the same surgeon. The following parameters were evaluated 1,3 and 6 weeks postoperatively: 1) Subjective discomfort index (SDI), 2) Tear break-up time (TBUT), 3) Central corneal thickness (CCT) and 4) Central Corneal Sensitivity (CCS).

Results:

Both groups demonstrated significant reduced CCS values and significant increased CCT values at all and the first examination point, respectively. Regarding TBUT, study group demonstrated significant better values in comparison to control group pre and post-operative. TBUT demonstrated significant negative correlation with FBS, which was significant better in study group. SD was significant better in study group only at the first week. FBS reduced to 29.34% in study group and it was the primary contributor of SDI in control group. FB reduction in the study group was the fundamental cause for a 1.45 times reduced discomfort sensation with Systane ultra.

Conclusions:

Taking into account the additional cost of Systane Ultra, our outcomes suggest that it should be used as a routine postoperative medication at least for 6 weeks since it significantly improves postoperative discomfort.
