Leap from bifocals to trifocals

Abstract Details

Purpose:

To compare the efficacy of trifocal intraocular lens versus bifocal Intraocular lens implantation in improving visual function after cataract surgery.

Setting:

Department of Ophthalmology, All India Institute of Medical Sciences, Jodhpur

Methods:

A total of 102 eyes undergoing cataract surgery were divided into two groups depending on the IOL implanted: a bifocal IOL implanted group in 80 eyes of 46 patients and a trifocal IOL implanted group in 22 eyes of 17 patients. The patients were evaluated post operation at 6weeks for uncorrected distance visual acuity, uncorrected intermediate visual acuity, uncorrected near visual acuity, spectacle independence, patient satisfaction and contrast sensitivity. The patients were secondarily evaluated for residual sphere, spherical equivalent (SE) cylinder and complications.

Results:

Both mean uncorrected and corrected VAs at far to intermediate distances were significantly better in the trifocal group than in the bifocal group (P≤0.03), while uniocular near VA did not differ significantly between groups. Average gain of 3 lines was noted in Snellen’s visual acuity chart for distance visual acuity. There was a statically significant difference between the two groups in residual cylinder (WMD: 0.11, P = 0.02), with trifocal showed significant better than bifocal IOLs. However, no significant differences were observed in UNVA, spectacle independence (p = 0.08), patient satisfaction, residual sphere or complications.

Conclusions:

Trifocal IOL technology had a clear advantage over bifocal IOLs in intermediate visual acuity. Trifocals are upcoming modality for the management of presbyopia. They are a magnificent addition to the armamentarium. However, bifocals still remain cost effective management in patients with moderate demands.
