Spherical equivalent outcome with Bausch and Lomb EyeCee One monofocal intraocular implant (IOL) in a large London teaching hospital.

Abstract Details

Purpose:

Cataract surgery requires careful planning of the post-operative refractive outcome estimates. Therefore it is vital that we know what our actual post-operative outcomes are following monofocal IOL implantation; this will improve accuracy in pre-operative planning. The purpose of our study is to assess the spherical equivalent outcomes with the Bausch and Lomb EyeCee One Monofocal intraocular lens implant (A-Constant = 119.7) used in routine phacoemulsification surgery. A benchmark standard of 85% of patients achieving a final spherical equivalent within 1 Dioptre (D) of the predicted figure and 55% of patients within 0.5 D is adopted for our study.

Setting:

Our study was conducted in a multi-diverse Ophthalmology department in a central London hospital.

Methods:

We conducted a retrospective cohort study between January and July 2019. Included were all routine phacoemulsification surgery with monofocal intraocular lens implant (Bausch and Lomb). Main parameters collected included: planned post-operative spherical equivalent outcome (D) based on intraocular lens selection pre-operatively using Zeiss IOL master and actual post-operative spherical equivalent outcome (D) based on auto-refraction at clinic follow up. Our data were collected using an electronic patient record software, Medisoft. Analysis performed using Microsoft Excel.

Results:

One hundred eyes (49:51 Left: Right) from patients aged 26-89 years old (49:51 Male: Female) were analysed. Median planned post-operative spherical equivalent outcome was -0.2D (range -0.56 to +0.08). Median actual post-operative spherical equivalent outcome was -0.625D (range -2.125 to + 0.625, p<0.00001). There were 4% of cases who had higher than +0.25D actual spherical equivalent. There were 22% who had a higher than -1D actual spherical equivalent. In 86% of cases there was a myopic actual spherical equivalent outcome. Regarding our benchmarks: 87% of cases achieved within 1D of predicted figure and 54% achieved within 0.5D of the predicted figure.

Conclusions:

Our study demonstrates that the majority of patients in our study had a myopic outcome with the Bausch and Lomb EyeCee One Monofocal intraocular lens implant. With an approximate -0.625D median post-operative outcome, our pre-operative selection should therefore be on the more emmetropic side of the IOL selection sheet. This study therefore will guide clinicians in choosing intraocular lens implants pre-operatively in cataract surgery thus helping to reduce refractive surprise.
