Anterior shift of monofocal IOL in pseudophakic eyes

Abstract Details

Purpose:

Normally, pseudophakic emmetropic patients will need at least +2.0D of more for clear near vision, but it is observed that patients implanted with monofocal IOLs can read at near distance with no or low addition of (+) glasses. Implanted IOL cannot mould during accommodation. Then it has to be shifting. This study is undertaken to find out whether monofocal IOL shifts during accommodation.

Setting:

This study was conducted at Shroff Eye Hospital, Navsari – India.

Methods:

142 pseudophakic patients implanted with either foldable or rigid monofocal IOLs were included in this study (94 Foldable & 48 Rigid IOLs). At the end of one month, after subjective correction for distance, they were asked to read near vision chart. Readings were taken when patient achieved N6 first with lowest +D lenses and till they maintained N6 with highest +D lenses. Anterior chamber depth was measured on anterior segment OCT with distant vision corrected glasses. Then one by one +1.0D, +2.0D, & +3.0D lenses were placed in front of eye and anterior chamber depth was recorded.

Results:

Anterior chamber depth was 3.24 mm (mean) with distant vision corrected glasses and without any near addition. With +1.0D it was 3.32 mm (mean), with +2.0D it was 3.38 mm (mean) & with +3.0D it was 3.44 mm (mean). It was also revealed that anterior chamber became deeper with addition of + lenses. The mean difference in ACD with +3.0D and without any near addition was 0.20 mm, which suggests that during near vision, zonular fibers work and push the IOL slight anteriorly making it of more higher value.

Conclusions:

Anterior chamber depth in 142 pseudophakic patients was measured by Anterior Segment OCT without and with +1, +2 & +3 D lenses. It was revealed that ACD became shallow (by 0.20 mm) when there was no near addition, which suggests IOL shifts slight anteriorly during near vision.
