New monofocal intraocular lens of improved depth of focus following explantation of multifocal and extended depth of focus intraocular lenses

Purpose:
To evaluate postoperative outcomes in two cases where a new monofocal intraocular lens (IOL) of improved depth of focus was used following explantation of multifocal and extended depth of focus (EDOF) IOLs.

International Vision Correction Research Centre (IVCRC), Department of Ophthalmology, Ruprecht-Karls-University of Heidelberg
Methods:
Analysis of two cases of IOL exchange, where ICB00 IOL (Johnson & Johnson Vision) was used following explantation of a multifocal IOL (LENTIS MPlus MF30, Oculentis, Case 1) and EDOF IOL (LENTIS Comfort, Oculentis¸ Case 2). Postoperative examinations included distance, intermediate and near visual acuity, defocus curve from -3D to +2D and evaluation of photic phenomena perception using Halo and Glare Simulator (Eyeland Design Network GmbH).
Results:
Case 1: 62-year-old male with monocular CDVA of 20/20, complaining about extreme photic phenomena. 2 weeks postoperatively monocular UDVA, UIVA and UNVA were 20/20, 20/32 and 20/50, respectively. Monocular visual acuity was ≥20/32 in the range between -1.0D and +2.0D. The patient reported a marked reduction of glare (size 29 vs. 64, intensity 12 vs. 86). Case 2: 81-year-old female complaining about cloudy vision with monocular CDVA of 20/25 and IOL opacification. 5 months postoperatively monocular UDVA, UIVA and UNVA were 20/25, 20/20 and 20/32, respectively. In defocus curve, monocular visual acuity was ≥20/32 between -1.0D and +1.5D.
Conclusions:
Since IOL fixation is never optimal in cases of IOL exchange, monofocal IOLs are used as they are tolerant against certain amounts of decentration. However, exchange of multifocal and EDOF IOLs results in the dilemma of only partial visual function restoration. The monofocal ICB00 IOL of improved depth of focus is a new option, demonstrating relatively good visual acuity at intermediate distance and some extent of refractive forgiveness.
