Diffractive-refractive and aberrometric extended depht of focus (EDOF) intra ocular lens (IOL): comparison of objective and subjective visual results after bilateral implantation

Abstract Details

Purpose:

To analyze vision acuity and subjective and objective quality of vision after bilateral implantation of either a diffractive-refractive IOL or an aberrometric IOL for the correction of pseudophakic presbyopia.

Setting:

In this perspective comparative study we enrolled 40 patients affected by bilateral cataract at the department of Ophthalmology of the Verona University Hospital and divided them into two groups. 20 patients received bilateral aberrometric EDOF IOL cataract surgery whereas 20 patients had a bilateral implantation of a refractive-diffractive EDOF IOL.

Methods:

We evaluated each patients at 3-month after surgery recording 4 meters best corrected and non-corrected visual acuity, near and intermediate (80, 67, 40 cm) best distance corrected and non-corrected visual acuity, defocus curve, contrast sensitivity, aberrometry, reading speed, reading acuity, halometry, optical quality and National Eye Institute Refractive Error Quality of Life Instrument 42 (NEI-42) questionnaire.

Results:

The study groups showed equivalent distant visual acuity. The aberrometric IOL group performed better on intermediate and near distances as during the myopic defocus and reading capability scores. There were no differences in vision quality considering the MTF-cut off and strehl ratio. Halometry has shown a tendency of diffractive-refractive IOLs to induce less halos. Both groups achieve good patient satisfaction results.

Conclusions:

The implantation of both EDOF IOLs allows high levels of patient satisfaction and quality of life. Visual acuity was satisfying at all distances tested, although aberrometric IOLs obtain results that are slightly better in the medium and near distance.
