Femtosecond laser-assisted removal of conjunctival melanocytic lesions

Abstract Details
Purpose:
To report on a series of patients with conjunctival pigmented lesions, who underwent excision and conjunctival autograft (CAG) harvest using the femtosecond laser (FSL)
Setting:
Tertiary eye centre
Methods:
Four eyes of four patients were included. All pigmented lesions were resected using the Ziemer Z8 FSL at 60 μm depth. Energy was set to 150% for the horizontal conjunctival stromal lamellar cut and 130% for the side cuts. Sizes of resection ranged from 5.0 x 5.0 mm to 5.5 x 7.0 mm. Oversized CAGs were resected in a similar fashion with the FSL at a depth of 60 μm and glued onto the resection defects. Slit lamp photography was taken at one month and one year after surgery and cosmetic outcomes were graded by a masked observer.
Results:
All the conjunctival pigmented lesions and CAGs were removed without difficulty or residual conjunctival stromal adhesion. No other complications were encountered intraoperatively or postoperatively. All patients achieved good to excellent cosmesis after one month. Cosmetic results remained stable without any lesion recurrence at one year after surgery.
Conclusions:
The FSL can be safely used for controlled resection of conjunctival melanosis and conjunctival nevi as well as preparation of ultrathin CAGs. Favorable clinical outcomes and excellent cosmesis may be attained as early as one month postoperatively.
