Docetaxel-related optic neuropathy in a patient with breast cancer

Abstract Details
Purpose:
To report a case of docetaxel-related optic neuropathy in a patient with breast cancer.
Setting:
Taipei City Hospital, Taipei, Taiwan.
Methods:
A case report.
Results:
A 44-year-old lady presented to our clinic due to blurring of vision in both eyes for 3 weeks. She had history of right breast invasive ductal carcinoma. And before the visual symptoms occurred, she just received a course of chemotherapy including the third cycle of docetaxel. The visual acuity with correction was 6/7.5 in the right eye, and only light perception in the left eye. The fundus exam and visual field test results were compatible with bilateral optic neuropathy. Laboratory and image examinations did not reveal other causes of optic neuropathy. After discontinuation of docetaxel, her vision of the right eye improved but the vision of left eye remained stable.
Conclusions:
We proposed that docetaxel induced the toxic optic neuropathy in our patient. Studies showed taxane-related toxic optic neuropathy might result from vascular dysregulation or direct neurotoxicity. This rare but serious complication must be bear in mind by clinicians while administering docetaxel.
