To the question of the interrelation between accomodation disorders and the structure of the anterior chamber angle in patients with myopia

Purpose:
The aim of the study was to study the interrelation between changes in the anterior chamber structures, hydrodynamic disorders and the functioning of the eye accommodative apparatus in patients with myopia
Setting:
The S.Fyodorov Eye Microsurgery Federal State Institution, the Volgograd Branch
Methods:
A prospective study was performed in patients with myopia of various degrees with anterior chamber depth more than 3.6 mm according to optical biometry (44 patients with low myopia, 32 – with moderate myopia, 19 – with high myopia). All patients underwent extended ophthalmological examination including tonography, rigidometry, accommodation and ultrasound biomicroscopy of the anterior segment.
Results:
Elements of goniodysgenesis such as anterior attachment of the iris and partial absence comb ligaments in 1-3 quadrants were revealed by gonioscopy in patients with low (6 eyes – 13%), moderate (8 eyes – 25%) and high myopia (11 eyes – 57%). The combination of increased corneal-compensated intraocular pressure more than 21.0 mmHg was noted in the group with low (1 case – 2.3%), moderate (3 cases – 9.4%) and high myopia (6 cases – 31.5%), which is described in the literature as ophthalmic hypertension syndrome with impaired accommodation
Conclusions:
The change of the anterior segment structures was noted with the indentation of the anterior chamber by myopic refraction. Stretching of the eye anterior segment is more expressed in patients with high myopia and it is more often accompanied by ophthalmic hypertension syndrome with overstretched accommodation
