Abstract Details
Purpose: 
To demonstrate the safe and effective way  of management of LASIK free cap involving the center of pupil . 
Setting: 
 Female patient was posted  for LASIK .During surgery of right eye she moved her head violently resulting in irregular free cap involving centre of pupil. The flap was repositioned and BCL applied . Other eye was operated without any complication at same setting. 
Methods: 
 Female pt with refraction of  OD -6 -1.25 @180   OS -4.5  02/20/ 2018 was posted for LASIK  During preparing the flap of right eye  with 90 micron SBK  blade the patient moved her neck violently resulting in a irregular  free cap involving the pupilary center . The flap was re positioned and bandage contact lens was applied . 
The BCL of rt eye was removed on 3rd postoperative day .The right eye was operated with PRK and 0.02% MMC application 45 days after free cap and  BCL applied for 4 days till the epithelium was healed. 
Results: 
 Her vision on 60th day post PRK was 6/6(P) with 0.5 D cyl at 180 degrees in rt eye, Which was her best corrected visual acuity preoperatively. In left eye her vision was 6/6 unaded. 
Conclusions: 
 From the visual outcome we can conclude that with proper management of irregular free capinvolving centre of pupil and with PRK after 45 days good results can be achieved.
