Data analysis of the outcomes of a large international sample of small incision lenticule extraction treatments for the correction of myopia and myopic astigmatism

Purpose:
To report on the clinical outcomes of over 3000 myopic Small Incision Lenticule Extraction (SMILE®) treatments carried out in an international hospital group over two European countries.
Setting:
Optegra Eye Hospitals, UK and Poland
Methods:
A retrospective audit of clinical outcomes of consecutive patients following Small Incision Lenticule Extraction (SMILE®, Zeiss) surgery for the correction of low and high myopia with or without astigmatism. Inclusion criteria were SMILE patients with a preoperative uncorrected distance visual acuity (UDVA) of 6/9 or better and with a targeted postoperative spherical equivalent refraction (SEQ) between 0.00D to -0.50D. Data audited were recorded in an electronic medical record typically at 1 and 3 months post-operatively.
Results:
Data from 3032 eyes of 1633 patients (mean age 33 ± 6 years) and a pre-operative SEQ of -4.62 ± 2.33D were included. All surgeries were uncomplicated. Mean LogMAR postop UDVA was -0.05 ± 0.1 and 94 % of all eyes achieved a postoperative UDVA of 20/20 or better with 92% of eyes achieving a postoperative mean SEQ within ±0.50D of the predicted target. 98% of patients achieved binocular UDVA of 20/20 or better. Eyes with >2D of preoperative astigmatism achieved a postoperative UDVA of 20/25 or better in 90% of cases.
Conclusions:
Small Incision Lenticule Extraction has been shown to be a safe and efficient procedure in myopes as this large-sample study confirms. Our results demonstrated no significant differences between visual acuity or refraction outcomes of low and high (>2D) astigmatic eyes.
