Purpose:
To evaluate the possibility of using anterior segment optical coherence tomography angiography (OCTA) in the fitting of scleral contact lenses.
Setting:
Moscow Research Institute of Helmholtz
Methods:
Fitting scleral contact lenses with a diameter of 15.6 mm with a sagittal height from 4500 to 5400 mm in 15 patients (24 eyes) with keratoconus stage III – IV we evaluated the apical clearance by means of optical coherence tomography of the anterior segment RS-3000 Advance 2 AngioScan (Nidek, Gamagori, Japan). Using the OCTA option of the same device we evaluated the effect of the lens edge on blood flow in the perilimbal region.
Results:
The apical clearance was in the range of 250-290 microns. Using OCTA we evaluated the effect of the peripheral part of the lens on the vascular flow in the perilimbal zone, in some cases noting its significant decreasing though there were no visible signs biomicroscopically. The decrease or absence of perfusion indicated the necessity to model the limbal zone of the lens and its edge as it could cause complications when wearing a scleral contact lens.
Conclusions:
The fitting of scleral lenses may be optimized by using OCTA which will allow the practitioner to perform the fitting with better control of the landing of the peripheral zone of the lens, taking into account its impact on the vascular structures of the anterior segment of the eye, and reduce the risk factors associated with impaired perfusion in the perilimbal zone.
