Modern possibilities of micropulse technology in secondary glaucoma treatment in patients after keratoplasty

Purpose:
To evaluate the micropulse cyclophotocoagulation (mCPC) effectiveness by secondary glaucoma after keratoplasty.
Setting:
S.N. Fyodorov Eye Microsurgery Federal State Institution of the Russian Ministry of Health
Methods:
The investigation included 5 patients (5 eyes) with a secondary glaucoma after penetrating keratoplasty (3 cases) and Descemet membrane transplantation (2 cases). All patients undergone anti-glaucoma surgeries from 3 to 18 months ago. A pre-op IOP with hypotensive drops laid between 27 and 35 mm Hg. The BCVA varied from 0.01 to 0.1. All patients undergone a micropulse CPC with «Cyclo G6 Glaucoma Laser System» (IRIDEX, USA), its power was 2000 mWt, exposition – 160 sec (80 sec for each hemisphere) and duty cycle – 31.3%. An anti-inflammatory therapy was prescribed in the post-op period.
Results:
An IOP on the first day post-op was 21-28 mm Hg, in one month post-op – 18-24 mm Hg. In 6 months after mCPC an IOP varied from 16 to 25 mm Hg with the hypotensive therapy. The visual acuity (VA) stayed at the pre-op level, in one case there was an increase of VA up to 0.2 due to the edema reduction.
Conclusions:
The micropulse CPC is possible in patients with a secondary glaucoma after keratoplasty. A longer and wider investigation is needed.
