Comparison of combined deep sclerectomy with trabeculectomy and trabeculectomy alone for pseudoexfoliation glaucoma

Abstract Details
Purpose:
To evaluate the efficacy and safety of deep sclerectomy combined with trabeculectomy (DST) in patients with pseudoexfoliation glaucoma (PEX) and to compare the results with conventional trabeculectomy (T).
Setting:
Tertiary Care Eye Hospital
Methods:
This was a retrospective comparative study. Only adult eyes with PEX were selected. 18 eyes underwent DST (Group I) and 20 eyes Trabeculectomy (Group II). All surgeries were performed by single surgeon. Post-operative intraocular pressure (IOP) reduction, visual outcomes, complications and need for anti-glaucoma medications (AGM) were assessed. Minimum followup period of 1 year. Complete success was defined as IOP < 22mm hg without AGM and qualified success as above criteria with AGM.
Results:
Mean preoperative IOP 28.5 ± 5.01 mmHg in DST group and 30.6 ± 4.6 mmHg in T group (p = 0.43) which reduced to 16.6 ± 3.60 and 18 ± 3.02 mm hg respectively. mean number of post-operative AGM at 12 months was less than 1 (DST = 0.62 ± 1.22, T = 0.72 ± 1.12). Group 1 (DST) had complete plus qualified success of 87.9% at 1 year while group 2 (T) had 85.2 %. DST group had 4 complications [DM perforation(1), hyphema (2), shallow AC(1)]. While 3 eyes group 2 complication [Hyphema (2 eyes), shallow AC (1 eye)].
Conclusions:
Deep Sclerectomy combined with trabeculectomy appears to have better efficacy in terms of IOP reduction with similar safety profile as compared to trabeculectomy alone. Small sample size and shorter duration of follow-up are limitations of this study.
