Comparison of deep sclerectomy and combined deep sclerectomy with trabeculectomy surgery for primary open angle glaucoma

Abstract Details

Purpose:

To compare efficacy and safety of deep sclerectomy (DS) vs. combined Deep Sclerectomy with trabeculectomy (DST) in patients with primary open angle glaucoma (POAG)

Setting:

Tertiary Care Eye Hospital

Methods:

This was a retrospective comparative case series. 63 eyes of adult patients with POAG were selected. 31 eyes (31 adult patients) underwent DS and 32 eyes DST. IOP reduction, visual outcomes, complications and need for anti-glaucoma medications (AGM) were assessed. Minimum follow-up of 1 year. Success was defined as IOP > 5mm hg and <22mm Hg.

Results:

Post operative IOP was lower in DST group vs. DT ( 16.82 +/- 4.54 mm Hg vs 18.25 +/- 4.90 mm Hg) difference was not significant (p: 0.261). Percentage mean reduction IOP more in DST ( 56.6 +/- 8.90 % vs. 51.54 +/- 10.9%) and the difference was significant (p<0.001). mean number of post-operative AGM in both at 12 months was less than 1 (DST = 0.52 ± 1.17, DS = 0.72 ± 1.12) (p: 0.627). Hyphema ( 3 eyes vs. 0) , shallow AC ( 2 vs. 0) and hyperfiltering bleb ( 2 vs. 0) most common complications.

Conclusions:

Combined surgery (DST) has better efficacy in terms of IOP reduction than DS and requires less additional post-operative AGMs. Saftey profile was better in DS with fewer complications.
