Two years of clinical outcomes after SMILE, LASEK and LASIK flap creation with femtosecond laser-assisted or mechanical microkeratome-assisted for myopia by generalized linear mixed models analysis

Purpose:
To compare the clinical outcomes of a large sample of myopia after subjects who had undergone SMILE, LASEK and LASIK flap creation with femtosecond laser-assisted (FS) or mechanical microkeratome-assisted (MM) for two years
Setting:
Beijing Aier-Intech Eye Hospital, Beijing, China.
Methods:
A retrospective comparative study. All patients recruited had a preoperative spherical equivalent (SE) refraction up to -16.75.0D myopia and myopic astigmatism from -0.25 to -6.0D. Clinical outcomes were recorded at 1day, 1week, 1, 3, 6,12 and 24 months postoperatively. A Generalized Linear Mixed Model was used to compare uncorrected visual acuity (UCVA) LogMAR, the refractive outcome of spherical equivalent (SE) and cylinder, mean K value, HOA RMS3 and HOA RMS5 in the four methods of surgery, the baseline was adjusted by analysis of covariance (ANCOVA) to compare the result of 24 months.
Results:
The study enrolled 8900 patients (8900 right eyes). 3296 patients in SMILE group, 3069 patients in FS-LASIK group, 966 patients in LASEK group and 1585patients in MM-LASIK, The mean attempted SE was -5.27±1.76D in SMILE, -5.73±2.47D in FS-LASIK, -7.45±3.08D in LASEK and -5.44±1.94D in MM-LASIK (P﹤0.001). At 2 years postoperatively among the four groups by adjusting the baseline of sex, age, and SE, uncorrected visual acuity (UCVA) LogMAR (P=0.514), mean K value (P=0.689), HOA RMS3 (P=0.342) and HOA RMS5 (P=0.114) were no statistical differences; while the value of sphere (P=0.004) and spherical equivalent (SE) (P﹤0.001)was a significant difference.
Conclusions:
SMILE, LASIK flap creation with FS or MM had similar efficacious procedures for the correction of myopia and outcome in UCVA, mean K, HOA RMS3, and HOA RMS5; while LASEK group performed lower refraction in two years postoperatively than SMILE, FS-LASIK, and MM-LASIK group.
