Clinical evaluation and prevalence of endophthalmitis in a reference hospital

Purpose:
The present study aimed to perform clinical analysis and identify the prevalence of endophthalmitis in a public service in Pernambuco, Brazil.
Setting:
Retrospective observational study at Altino Ventura Foundation in Recife, Pernambuco, Brazil.
Methods:
Study conducted by reviewing the medical records of patients who presented the diagnosis confirmed of endophthalmitis at the Altino Ventura Foundation in the period between January 2015 and December 2018. Socio-demographic and clinical variables were analyzed including visual acuity (VA), etiological agent, cause, history of ocular surgery, and number of patients that evolved to evisceration.
Results:
The medical record of 224 patients were evaluated (122 male). The mean age was 56.6 ± 21.4 years (range, 1-95 years). History of ocular surgery (93 patients), trauma (44 patients), and endogenous etiology (39 patients) were the main events that led to endophthalmitis. Vitreous samples were collected in 118 eyes (56 were negative). Coagulase negative staphylococcus was the most frequent agent (19 cases). Klebsiella sp (n=7) and Streptococcus pneumoniae (n=3) resulted in the worse visual prognosis with all cases resulting in a VA worse than movement of hands (p = 0.015). About 71 patients were submitted to ocular evisceration.
Conclusions:
Cataract surgery was the main cause of endophthalmitis in this study. Klebsiella sp and Streptococcus pneumoniae were related to worse prognosis.
