Purpose:
purpose of this study is to report a case of choroidal tuberculoma in ayoung patient with pleural effusion.
Setting:
vardhaman mahavir medical college and safdarjung hospital,new delhi,india
Methods:
21year old female with left pleural effusion on treatment for 4 months presented with diminution of vision in right eye for the past 6months.vision was finger counting close to face with accurate projection of rays with relative afferent pupillary defect grade2. fundus showed elevated yellowish lesion in the macula of more than 1disc diameter withsurrounding retinal edema.optical coherence tomography showed subfoveal neurosensory detachment with intraretinal fluid and thickened choroid.fundus fluorescence angiography showed leakage around tuberculoma.patient started on T.prednisolone 10mg/kg/day with tapering according to clinical response along with continuation of antitubercular treatment.
Results:
edema resolved with vision improving to finger counting at 2meters with improvement in the color vision and contrast within one and half months of the treatment.
Conclusions:
oral steroids along with atitubercular treatment can reduce the lesion size and edema and improve the vision and contrast sensitivity thereby improving overall clinical picture.
