Diverse clinical presentations of ocular melioidosis

Abstract Details
Purpose:
To report the first two cases of ocular melioidosis in Sri Lanka, that had diffferent clinical presentations.
Setting:
National Eye Hospital, Colombo: a tertiary care eye hospital in Sri Lanka.
Methods:
Case series
Results:
Case 1: A 43 year old with right eye visual loss for two days. Visual acuity was 6/36; mutton fat keratic precipitates, anterior chamber cells and vitreous haziness was seen. An elevated CRP and melioidosis serology of 1:10240 were seen. Pars plans vitrectomy was performed; ocular inflammation subsided and the patient had a BCVA of 6/24+.
Case 2: A 34 year old with visual loss of left eye for 3 days. Vision was NPL; he was febrile, with orbital cellulitis and panophthalmitis. Neutrophil leukocytosis, elevated CRP and a melioidosis serology of 1:5012 were noted. Enucleation of L/eye was preformed.
Conclusions:
Ocular Melioidosis usually has poor visual outcomes; early diagnosis is difficult due to inconsistent ocular manifestations and inconclusive laboratory tests. A high degree of suspicion is warranted in endemic regions.
