Abstract Details
Purpose:
To compare surgical outcomes of combined procedure Descemet’s endothelial keratoplasty (DMEK) and cataract surgery with versus without using ocular viscoelastic device (OVD).
Setting:
Dep of Ophthalmology, University Hospital Kralovske Vinohrady, Prague, Czech Republic
Methods:
The prospective randomized controlled study comprised 66 eyes of 55 patients who underwent DMEK and phacoemulsification with implantation of monofocal IOL in one single procedure. 33 eyes underwent the standard surgery with using OVD (group 1) and 33 eyes without any use of OVD (group 2). All patients had corneal endothelial dystrophy (Fuchs) and cataract. Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), immediate and early postoperative complications were evaluated
Results:
After 3 months the mean postoperative UDVA in group 1 was 0,6 +- 0,3, in group 2 was 0,5 +- 0,2. CDVA was 0,8 +- 0,4 in group 1 and 0,8 +- 0,1 in group 2. 6,1% of eyes in group 1 and 21 % in group 2. There was one complication perioperatively due to the mature cataract in group 2. The anterior vitrectomy with IOL implantation to ciliary sulcus was done.
Conclusions:
According to our results, combined procedure DMEK and cataract surgery with using the ocular viscoelastic device does not seem to be associated with higher number of rebubbling or other postoperative complications
