Purpose:
Case series interventional study presenting the management of Lasik flap interface fluid syndrome after Descemet Membrane Endothelial Keratoplasty (DMEK) surgery
Setting:
Case series interventional study. 5 cases.
Methods:
5 cases were included in this study, all patients had a history of laser in situ keratomileusis (LASIK) surgery and undergo DMEK due to bullous keratopathy. After uneventful surgery, all patients presented with corneal edema and fluid interface under the LASIK flap, which was demonstrated with anterior segment optical coherence tomography (AS-OCT). Normal intraocular pressure was measured in all eyes. Administration of pressure-lowering agents did not reduce corneal edema or the amount of fluid in the flap.
Results:
In 4 of the 5 cases, the LASIK flap was lifted, the fluid was removed, and the flap was placed again. Significant improvement in the corneal appearance, thickness and in the corrected distance visual acuity was shown in all 4 cases. AS-OCT showed resolution of the flap interface. In one case, fluid was absorbed without surgical intervention, improving corneal thickness and visual acuity.
Conclusions:
The possibility of fluid interface syndrome as a complication of DMEK surgery should be taken into consideration.
In 4 of the 5 cases presented, conservative treatment was not helpful, however, draining the fluid interface was very effective. We recommend performing AS-OCT to monitor the endothelial attachment and the flap interface in DMEK patients which have a history of LASIK
