Structural arrangement of collagen fibrils in keratoconus, suspect keratoconus and normal eyes

Abstract Details
Purpose:
To study the changes in collagen fibril structures in keratoconus (KC), suspect keratoconus normal eyes using custom built Ultrahigh resolution polarization sensitive (PS) – OCT.
Setting:
Narayana Nethralaya, India
Methods:
50 normal eyes, 25 suspect KC eyes and 25 eyes of varying grades of KC were imaged to analyse the collagen fibril structure with PS-OCT. A scan field of 11mm diameter was achieved using conical scan setup enabling uniform signal strength throughout the cornea. A concise representation of corneal fibril distribution was generated using Phase retardation and axis orientation enface maps. To ensure repeatability all eyes were imaged at least 3 times.
Results:
Enface maps of the normal eyes showed preferential arrangement of collagen fibril structures. Suspect KC and KC eyes showed abrupt changes in retardation and axis orientation information as compared to normal eyes. Binding of collagen fibrils appeared to be weak in KC eyes in regions of higher curvatures.
Conclusions:
The binding density of collagen fibrils was high in normal eyes, followed by suspect KC eyes and sparse in KC eyes. Polarization sensitive data indicates suspect eyes had a trend similar to early keratoconus. Further, Posterior corneal surface enface maps derived from PS-OCT can be used to reliably to distinguish between KC and Suspect KC. Polarization sensitive data can also be used to track progression of KC from normal and suspect KC stages.
