Functional and structural evaluation of the meibomian glands in thyroid eye disease

Abstract Details

Purpose:

To examine morphologic changes in the Meibomian glands associated with patients with Graves orbitopathy (GO) using infrared noncontact meibography system (Sirius, CSO, Italy) and to evaluate its correlation with GO clinical manifestations.

Setting:

Health Ministry University Hamidiye Etfal Hospital, Ophthalmology Department, İstanbul, Turkey.

Methods:

This retrospective study examined 35 GO patients and 30 normal control participants. The structure of the meibomian gland was assessed using the meibography imaging system of Cirius Corneal Topography. The tear film break-up time (BUT), Schirmer 1 test were measured; clinic activity scores (CAS), exophthalmos and eyelid retraction were also assessed.

Results:

The average of BUT and Schirmer test values were 12,81 sec. and 16.50 mm. The average of CAS was 0,58. There were no statistically significant differences between Schirmer and BUT test measurements in the GO and normal groups. Also the meibography scores did not differ between the patient and the control groups. In the linear regression analysis, the meibography score of the GO patients had a negative correlation with exophthalmos and palpebral fissure height.

Conclusions:

In our study although GO patients had severe symptoms of dry eye compared with the control participants, Schirmer , BUT and Meibography parameters were similar with the control group. Most of the GO patients were in the inactive period, and average value of the CAS were lower than 3 (p<0,05). Maybe these factors resulted similar outcomes between the patient and control groups. Still, using the meibography system, we can analyze the structural changes of Meibomian glands in GO patients.
