Effect of pterygium on corneal wavefront aberration: comparative study with normal fellow eyes

Purpose:
To evaluate the effect of pterygium on corneal wavefront aberration.
Setting:
Ophthalmic clinic, Seoul National University Hospital
Methods:
Patients who have unilateral pterygium and normal fellow eye were included. Anterior segment photography was used to measure the length, width, and area of the pterygium. Anterior segment optical coherence tomography was used to measure the thickness at the thickest point. Corneal wavefront aberrations were analyzed using corneal topographer system. Each topographic parameter (simulated keratometric values, astigmatism, irregularity, and corneal wavefront aberrations) from eyes with pterygium was compared with that of normal fellow eyes. Correlation analysis was performed to evaluate the relationship between optical parameters and the length, width, area, and thickness of the pterygium.
Results:
Thirty-nine pterygium and normal fellow eyes of 39 subjects were enrolled. Astigmatism, oblique/horizontal trefoil, horizontal coma, oblique/horizontal quatrefoil, and root mean square of 3rd and 4th order of higher order aberrations (HOAs) were significantly different between pterygium and normal fellow eyes. Most of the differences of astigmatism and HOAs were significantly correlated with pterygium length, width, and area but not with the thickness.
Conclusions:
Nasal pterygium, especially the length, width, and area, significantly affect corneal astigmatism and HOAs.
