Simple method of making rhexis to prevent extension in intumescent and morgagnian cataract

Abstract Details
Purpose:
Purpose is to develop a simple technique to debulk the elevated intra-lenticular pressure in cases of intumescent and morgagnian cataracts to prevent extension of rhexis towards periphery.
Setting:
This study was conducted at Shroff Eye Hospital, Navsari – India.
Methods:
In 48 cases of hyper mature intumescent and morgagnian cataracts, 26-gauge 0.5 inch long sharp hypodermic needle attached to 2 cc syringe was introduced in visco filled AC, through corneal side port. Then it was gently pushed through anterior capsule in the centre and assistant was asked to withdraw the fluid gently, keeping the needle absolutely steady. Then AC was refilled with visco and rhexis could be completed. Phacoemulsification and IOL implantation were done without any damage to rhexis margin.
Results:
In all cases 0.1 ml to 0.3 ml (mean 0.16 ml) of lenticular fluid could be aspirated. Rhexis could be easily completed in all cases from the puncture created by the tip of the needle. Phaco procedure could be done without any complication in all cases with insertion of appropriate hydrophobic foldable IOLs. Rhexis margin was not extended during phaco in any case. At the end of one month, eyes were quite and BCVA was from 6/9 to 6/6.
Conclusions:
Debulking by withdrawing of intra-lenticular fluid in visco filled eyes by this simple technique is found to be effective and safe method to make rhexis without fear of extension in 48 cases of intumescent and morgagnian cataracts.
