Abstract Details
Purpose: 
The authors present the case of a 50-year-old male patient with congenital aniridia, mature subluxated cataract, secondary glaucoma, hypoplasia of optic nerves and sensoric nystagmus in both eyes. We also present a novel surgical technique for managing an aphakic case with congenital aniridia undergoing transscleral fixation. 
Setting: 
 Department of Ophthalmology Professor K. Gibiński University Clinical Center, Medical University of Silesia, Katowice, Poland 
Methods: 
 Our technique is based on inserting polypropylene suture from one to another side of the sclera wall at the 4 and 8 o’clock meridians.
 The two sutures are inserted approximately 1,5 mm posterior to the limbus and they are parallel to each other to provide an adequate support for PCIOL stabilization. 
Results: 
 This technique of intraocular lens fixation  showed excellent stability, during whole surgery. There was no dislocation of the artificial lens and no severe complications during and after the procedure. 
Conclusions: 
 Patients with congenital aniridia remain a great surgical challenge and they require an individual therapeutic approach in every case. 
Our technique of IOL  fixation  provides a stable support for lens and it is a good  option to treat patients with cataract and comorbid aniridia.
